AHP funds WARM subrecipients across the state of Illinois. These agencies provide peer recovery support services using a warm handoff model to individuals with opioid use disorder or polysubstance use disorder and who are in housing transition.
All resource links below, including to the WARM SubHub, the Data Dictionary and Measurement Guide, the WARM Data Collection Excel, and the WARM Evaluation Data Collection form, link to the AHP-hosted WARM SubHub. To access these accounts, users need to belong to an agency that provides WARM services via an AHP-funded grant. If you fit this criteria but do not have an account on the WARM SubHub, please reach out to AHP via the help desk to request an account. Users will also need to belong to an agency providing WARM services to access the OASEE data portal.
