AHP funds BASE subrecipients across the state of Illinois. These agencies establish or expand innovative and evidence-informed programs for preventing, delaying, and reducing substance use among youth and emerging adults, ages 6-25.
All resource links below, including to the BASE SubHub, the Data Dictionary and Measurement Guide, the BASE Data Collection Excel, and the BASE Evaluation Data Collection form, link to the AHP-hosted BASE SubHub. To access these accounts, users need to belong to an agency that provides BASE services via an AHP-funded grant. If you fit this criteria but do not have an account on the BASE SubHub, please reach out to AHP via the help desk to request an account. Users will also need to belong to an agency providing BASE services to access the OASEE data portal.
